Recently, while driving my car, I've noticed a problem that has been getting progressivley worse

While driving, under normal acceleration it is fine, but if I try and accelerate at full throttle the rpm's jump up and the car dosent accelerate accordingly (it's almost like flooring it in neutral)

It also happens if i have the car in gear, it'll be at about 3,000rpms, I'll floor it to pass (in 2nd gear) and the rpm's will jump up but the car dosen't accelerate like it should, it's almost as if the clutch disc isn't catching the flywheel properly, but there is no shaking or jittering during acceleration (which I think would indicate a bad or worn clutch), so I really don't know what the problem would be
