How do you know if the wheel bearings need to be replaced?

How do you know if the wheel bearings need to be replaced?

Strange growling noises, roughness when turning wheel by hand while jacked up, looseness when grabbing wheel top and bottom and pushing in/out while jacked up, signs of grease coming out around hub, any or all of the above.
