This won't work on a 3sgte unless you figure out another way to push water into it the turbo. The only thing that it is missing that doesn't make it like the GTFour coolant neck is the turbo water outlet. Aside from this the sensor ports are the same size and so are the diameters of the TB line, radiator line, and heater hose. Also the sensors mount to the side facing the radiator. This came with the my Gen III head when I got a new one.
This is the bottom view of it, it's the best picture that I could get that shows almost all of the outlets in one shot as well as the side that mounts to the head. The only thing blocked is the water outlet to the TB.
